Spain's public healthcare system consistently ranks among the best in the world, and it's one of the reasons many Americans put Spain on their shortlist. But the way you access it depends on your visa route and your situation, and there's a sequence to get right.

Step one: private insurance for your visa

Every residence visa application requires proof of private health insurance from a Spanish-licensed insurer, with full coverage and no co-pays or waiting periods. U.S.-based travel insurance is not accepted. Your policy needs to be in place before your consulate appointment.

Step two: public healthcare once you're resident

The Spanish public system (SNS) is accessed through your residence card, your town-hall registration (empadronamiento) and Social Security. If you're working and contributing, access follows from that. If you're not (a retiree on the Non-Lucrative Visa, for example), many regions let you join the public system through the Convenio Especial, a monthly buy-in whose cost depends on your age. The rules vary by autonomous community.

Keeping a private policy anyway

Many Americans keep private insurance alongside public access for speed, choice of specialist and English-speaking doctors. Private cover in Spain is far more affordable than in the United States; the exact premium depends on age and the level of coverage.

The Medicare question

Medicare does not cover you in Spain. Some Americans suspend Part B to save the premium, but re-enrolling later can carry penalties. This is a decision to make deliberately, with advice, before you leave.
